Mystic Moon Tattoos is committed to ensuring the security and privacy of your personal information. We abide by federal and provincial data privacy laws as outlined by the Personal Information Protection and Electronic Documents Act (PIPEDA) and Personal Information Protection Act (PIPA), and we take additional steps beyond legal requirements to keep your information safe.

1. Information We Collect

When booking an appointment, we collect the following personal information through forms filled via Jotform:

  • Legal Name

  • Preferred Name

  • Phone Number

  • Email Address

  • Photo of Government-Issued ID

2. How Your Information Is Used

Your information is used only for the following purposes:

  • To verify your identity and age in compliance with the law and our policies to only tattoo those 18 or older

  • To contact you with important information regarding your appointments

  • To maintain accurate client records for health and safety purposes

  • To protect our staff and clients for liability purposes

  • To set up your client profile with our booking software, Vagaro

3. External Access to Your Information

We use your preferred name, phone number, and email address to set up your client profile in our booking software, Vagaro.

Aside from collecting your information via Jotform and setting up your profile with Vagaro, we will never sell or share your data with any third parties for any reason. Your information will never be used, sold, or shared for marketing purposes. Your information will never be used, sold, or shared for training AI models.

4. Protecting Your Information and Privacy

We use multiple methods to ensure your data is protected, including:

  • Local Storage: All personal information is deleted from Jotform within 7 days of submission. After that, it is stored locally on a secure device.

  • Encrypted Backups: All backups that include your personal information are securely encrypted and stored separately from our primary computer.

  • Access Control: Only two people, the shop owner/artist and the computer administrator, have access to your personal information. Both individuals adhere strictly to our privacy and security protocols to ensure your information is used only for the purposes listed in Section 2 (How Your Information Is Used.)

We are committed to protecting the confidentiality of your personal information. All data is stored securely and we take all reasonable precautions to protect your data from misuse, loss, and unauthorized access.

5. Data Retention and Deletion

We retain your information only for as long as needed for the purposes outlined above or as required by law, whichever comes sooner. If you wish to have your information deleted, you may contact us at any time to request secure deletion of your records. If we are legally required or bound by liability protections to retain your information for a longer period, we will inform you accordingly. The following information must be retained to protect clients and staff:

  • Artist's full name

  • Client's full name

  • Client's email and telephone number

  • Consent and policy agreement documents signed by clients

Our booking software, Vagaro, does not provide us access to manage client accounts directly. To delete or modify the information in your Vagaro client account, you must log into Vagaro using the email you originally provided us, or contact Vagaro directly.

6. Your Rights

You have the right to:

  • Request access to your personal information

  • Request that we update any inaccurate or outdated information

  • Request that we delete your personal information, subject to any legal obligations or liability protections that may require us to retain it, more details in Section 5 (Data Retention and Deletion)

All requests will be fulfilled within 30 days.

7. Data Breach Notification

In the unlikely event of a data breach that may compromise your personal information, we will notify you promptly and take the necessary steps to mitigate potential risks.

8. Third-Party Privacy Policies

  • Jotform: We collect your information through forms processed by Jotform. You can read their privacy policy here.

  • Vagaro: We use your preferred given and legal name, email address, and phone number to set up your client account with our booking software, Vagaro. You can read their privacy policy here.

9. Privacy Policy Changes

Any changes to our privacy policy will be applied to this page.
